No-Fault/Motor Vehicle Accidents
Our committed No-Fault Department supports our patients, by scheduling their appointments and providing care. When booking an appointment, patients should have the following motor vehicle information ready:
- Date of the accident
- Location of the accident
- Name of the no-fault insurance carrier
- Claim details, including the claim number and adjuster information
*Claim must be open - We will also need your Health insurance information and details of any emergency room visit, including the hospital name.
Documents Needed at Time of Appointment:
- Photo identification
- Completed New Patient Packet, if emailed. If not, it can be completed in the office.
- Health insurance card.
- Automobile insurance card and Police report – if available at the time of appointment.
- Declaration page of automobile insurance policy *must be provided prior to appointment.
- Discharge summary from the emergency room, together with any diagnostic studies performed
